You must submit this form to the Financial Aid Office if your FAFSA has been selected for the standard verification (V1) process. You will need to complete this form with the assistance of your parent. A pen to paper signature by your parent is required for processing. Please be aware federal financial aid cannot be released to your account until all verification requirements have been received and processed by our office.
