Identity & Statement of Educational Purpose

If you are in the greater Denver Metro area you will be expected to come to campus to complete this form.  If you live outside of the Denver Metro area, you can mail the form,  but please be sure to provide all necessary requirements. A notary will need to complete their section on the form itself and you'll need to include a copy of your unexpired, government-issued, photo ID.

*Please note:

• The 2024-2025 form is for the Fall 2024, Spring 2025, and Summer 2025 Semesters
• The 2025-2026 form is for the Fall 2025, Spring 2025, and Summer 2026 Semesters
