Identity Verification Requirement

To complete the Identity Verification requirement, you may either visit our office in person or upload your completed, notarized Identity Verification form through Student Forms. You must be prepared to provide a copy of a valid, unexpired U.S. passport or other government-issued photo ID for identity verification.

Once your documentation has been received, processing typically takes 3–5 business days. If additional information is needed, you will be notified through your preferred communication method in Student Forms.

Please note: The 2026–2027 Identity Verification form is valid for the Fall 2026, Spring 2027, and Summer 2027 semesters.
