Honors Program Completion Form

The Honors Program Completion Form can be completed by students who have completed a minimum of 15 credit hours AND have maintained a cumulative grade point average of 3.5 or higher.

As part of the Honors Program, you will be eligible to receive formal recognition. Please fill out and complete this form and turn it into Courtenay Avant, Honors Coordinator. This form has a deadline each year, please read it fully before completing.
