Disability Discharge

Disability Discharge

If you previously had federal student loans forgiven because of a Total and Permanent Disability (TPD), federal rules require an additional review before you can receive new financial aid or borrow new loans. This form helps us determine what type of aid you plan to use for the upcoming year.

How to Submit

  • Complete the Financial Aid Disability Discharge Form:

    This tells us whether you plan to borrow federal student loans or only use other types of financial aid (such as grants or scholarships).
  • If you plan to borrow federal student loans:
    You must also upload a completed Physician’s Certification of Borrower’s Eligibility to Engage in Substantial Gainful Activity, signed by a qualified physician. This certification is required before you can take out new loans.
  • If you do NOT plan to borrow loans:
    You do not need to submit the physician certification—just complete the form.

Who Can Complete the Physician’s Certification?

The certification must be completed and signed by a physician who is legally authorized to practice medicine in the United States.

This includes:

  • MD (Doctor of Medicine)
  • DO (Doctor of Osteopathy)

The following are not allowed to sign this form:

  • Nurse practitioners
  • Physician assistants
  • Clinical psychologists
  • Licensed therapists or counselors
  • Podiatrists
  • Any physician who is not licensed to practice in a U.S. state
